If TENS lets you move more comfortably, that can be useful. If it only masks pain while you keep aggravating the problem, it may be less helpful.

Cochrane reviews have found uncertainty around whether TENS reliably improves chronic pain compared with sham treatment or usual care. NCCIH also notes that evidence for TENS in low back and neck pain has been inconclusive in some reviews.

That does not mean TENS never helps. It means the evidence is not strong enough to promise reliable relief for everyone. For a low-risk tool, a careful trial can still make sense for some people.

How to Try TENS Safely

  1. Read the device instructions before use.
  2. Place pads on clean, dry skin around the painful area, not on broken skin.
  3. Start at a low intensity and increase slowly until it feels strong but comfortable.
  4. Use it during a calm activity first, not while driving, bathing, or sleeping.
  5. Stop if it causes burning, skin irritation, dizziness, unusual symptoms, or worse pain.

A common starting point is 15 to 30 minutes, but follow your device instructions and clinician guidance.

Who Should Ask a Clinician First

Do not start TENS without medical guidance if you:

  • have a pacemaker, implanted defibrillator, spinal cord stimulator, or other implanted electrical device
  • are pregnant
  • have epilepsy or seizure risk
  • have cancer in the treatment area
  • have reduced skin sensation where pads would go
  • have unexplained pain that has not been evaluated

Avoid placing pads across the chest, on the front of the neck, over the eyes, on broken skin, or anywhere the device instructions warn against.

How to Judge Whether It Is Working

Do not judge TENS by how interesting it feels during the session. Judge it by what it changes afterward.

Useful signs:

  • pain is lower for a meaningful period
  • movement feels easier
  • you sleep better
  • you use less guarding or bracing
  • it helps you do rehab exercises more comfortably

Not useful signs:

  • relief lasts only while the device is on and changes nothing else
  • skin gets irritated
  • you turn the intensity higher and higher without better results
  • pain is worsening overall

The Bottom Line

A TENS unit may help some people manage pain, especially as a short-term add-on to movement, rehab, and other conservative care. But evidence is mixed, and it should not be sold as a guaranteed fix.

Try it carefully, track whether it meaningfully changes your function, and ask a clinician first if you have implanted devices, pregnancy, seizure risk, reduced sensation, or unexplained symptoms.
