Pain after exercise can mean very different things. A dull ache the day after an unfamiliar workout is not the same pattern as a sudden sharp pain during a lift. Neither intensity nor toughness tells you which one you are dealing with. Timing, location, function, and the direction of change are more useful clues.
This guide cannot rule out an injury, but it can help you decide whether to recover, modify activity, or seek an assessment.
What Delayed-Onset Muscle Soreness Usually Looks Like
Delayed-onset muscle soreness, often called DOMS, follows an activity that was new, more intense, or more prolonged than your body was used to. According to the American College of Sports Medicine, it commonly begins 12 to 24 hours after exercise and may feel strongest between 24 and 72 hours.
Typical features include:
- a dull, tender, or tight feeling in muscles you trained
- soreness on both sides when both sides did similar work
- discomfort when the muscle is pressed, stretched, or used
- stiffness that improves somewhat after an easy warm-up
- a gradual improvement over several days
DOMS is not caused by lactic acid remaining in the muscle. It is associated with the response to unfamiliar loading and microscopic muscle disruption.
Features That Are More Concerning for Injury
An injury is more likely when there was a clear moment something went wrong or when function is meaningfully impaired. Watch for:
- sudden, sharp, tearing, or snapping pain during the activity
- rapid swelling or significant bruising
- a visible change in shape
- inability to bear weight or use the area normally
- marked weakness, instability, locking, or giving way
- pain focused in a joint, bone, or very specific point
- symptoms that keep worsening instead of beginning to settle
A "pop" is not automatically a serious injury, and some injuries occur without one. Treat it as one piece of the story, especially if it came with pain, swelling, or loss of function.

Timing
DOMS usually arrives later. Injury pain often starts during the movement or immediately afterward, although some overuse injuries build gradually.
Distribution
DOMS tends to occupy much of a worked muscle. Injury pain may be more focal, though this is not a perfect rule.
Movement
Mild soreness may loosen during ordinary movement. Injury symptoms may cause limping, compensation, instability, or a clear loss of range or strength.
Trend
Normal soreness should show an overall improving trend. Day-to-day variation is common, but continued deterioration, new swelling, or loss of function deserves more caution.
What to Do for the Next 24 to 48 Hours
If the pattern looks like ordinary soreness, keep normal daily movement comfortable, lower the intensity of training for the affected muscles, sleep, eat normally, and allow recovery time. You do not need to eliminate all activity, but repeating the same hard session before you have recovered is unlikely to help.
If an injury is possible, stop the aggravating activity and protect the area from loads it cannot currently tolerate. Ice may be useful for short-term comfort when a new injury is swollen or irritated, but it is not a diagnostic test. Avoid aggressive stretching, deep massage, or "testing" the injury repeatedly.

When to Return to Training
Do not use pain alone as the only test. A gradual return is more reasonable when everyday movement is close to normal, swelling is absent or settling, range of motion is improving, and a lighter version of the exercise does not cause a substantial or lasting increase in symptoms.
Start below your previous workload. Change one variable at a time, such as weight, repetitions, distance, or speed, so you can tell how your body responds.
When to Get Medical Help
Seek prompt care if you cannot bear weight, the area looks deformed, swelling is large or rapidly increasing, there is severe weakness, a limb becomes numb or unusually cold, or pain followed a major fall or collision. Get assessed if symptoms are not improving as expected, repeatedly return with the same activity, or affect normal sleep and daily function.
The Bottom Line
Ordinary muscle soreness tends to arrive later, affects the muscles that did unfamiliar work, and improves over several days. A sudden onset, major swelling, focal pain, instability, or loss of normal function shifts the picture toward possible injury. When the pattern is unclear, reducing the load and getting a qualified assessment is safer than trying to prove you can train through it.
